Ultrasound-guided injections provide cutting edge precision in the treatment of acute or chronic pain and injuries. Ultrasound is an imaging technology that utilizes sound waves rather than ionizing radiation. It is now being used extensively in the United States to diagnose and treat musculoskeletal problems since it allows for real-time visualization of extra-articular structures such as tendons, ligaments, muscle, bone, peripheral nerves, arteries, and veins.

Musculoskeletal Ultrasound used for the aspiration of joints, cysts, and tissues. It is often more diagnostic than MRI since real-time dynamic exams can be performed, it’s faster, it’s less expensive, and it’s easy to compare the opposite non-involved region of the body. It provides much more resolution than fluoroscopy for the fine detail of structures. Ultrasound resolution is 2.5 times greater than a MRI.


When nerves are “pinched” by scar tissue or tight muscles and connective tissue, they are basically being suffocated. There is decreased circulation, and therefore, the nerve doesn’t receive the appropriate amount of nutrition. This is thought to be the cause of nerve pain (neuritis). With the use of ultrasound imaging, the area of nerve impingement is visualized. A needle is then placed adjacent to the impingement, and a solution of anesthetic with saline fluid is injected to separate the nerve from the surrounding tissues. By releasing the pressure from the nerve, circulation is restored, plus there can be immediate relief of pain.


Stem Cell/Bone Marrow Aspirate is used for severely degenerated joints. BMAC is obtained from the patient’s hip bone, the iliac crest. An anesthetic is injected to make this procedure more comfortable. The aspirated bone marrow placed into a special centrifuge, which concentrates up to 1 million stem cells and growth factors. This concentrate is injected into the affected area. Typically only one treatment is necessary to stimulate the body to regenerate joint cartilage and may eliminate the need for invasive joint replacement surgery.


PRP is used for moderately degenerated joints and tendon and ligament sprain/instability. PRP is obtained with a simple blood draw. The blood is then placed into a special centrifuge, which concentrates the platelets, stem cells, and growth factors up to 500%. The platelet concentrate is then injected into the affected area. Typically one, occasionally, two treatments are required. PRP is a component of your blood (plasma) with concentrations of platelets above-average values. PRP usually contains 3-8 times the concentration of normal platelet levels. After an injury, platelets are on the front line of the healing response and play a critical role by releasing growth factors. These growth factors influence tissue repair in a variety of different cell types, including tendon, muscle, and cartilage cells.

The centrifuge systems produce a completely autologous platelet biologic with a high concentration of viable platelets, extracted from a small amount of the patient’s own blood, spun through a centrifugation process and resulting in a dense platelet-rich fibrin matrix (PRFM) that can be delivered directly to the injury/pain site and to potentially stimulate a reparative healing response for soft tissue and bone repair.

There is a growing body of evidence to support the use of autologous platelet-derived growth factors for a variety of medical conditions at this time. Currently, there is some evidence that PRP is effective for bone healing, cartilage repair, and wound healing. This therapy has been shown to help to heal tennis elbow and some other sports injuries. Currently, there are ongoing clinical trials assessing the efficacy of PRP for chronic Achilles tendinopathy, bone healing, knee cap pain, ankle sprains, plantar fasciitis, and numerous other musculoskeletal injuries.
